I too am seeing the same behavior (for me, it's SQLDeveloper, an application from Oracle, that's not in my index. I haven't checked to see if others are missing.)I'm running LB 6.1.2.

I would quit LaunchBar and delete it’s caches by deleting the folder ‘at.obdev.LaunchBar’ in /Users/your-name/Library/Caches/. Then restart LaunchBar and it will completely reindex all items.
